Have you had known contact with a confirmed or suspected case of COVID-19 in the past 14 days.
Have you recently returned from overseas travel or any known hotspot areas?
Are you or anyone in your household awaiting COVID-19 test results?

When you arrive for your appointment, we ask that you:
Arrive wearing a mask (as per the NSW rules and restrictions for visiting non-residential premises in NSW)
Sanitize your hands
Scan in with a COVID-QR code
Maintain social distancing
